Dr. Minera´s Personal Story

Over and over, I found myself helping patients recover after a fall. I met Beth at the Skilled nursing after her surgery; she had fallen a few days earlier in a parking lot, catching her foot on a concrete block on the way to her usual Thursday lunch with friends that brought her so much joy. She felt a sharp pain in her knee and left hip, and later learned she had fractured the neck of her femur. What worried her most was not just the surgery, but the fear that she might not be able to walk confidently again or keep up those lunches with her friends. I knew the real problem was not just a broken bone, but the loss of independence, strength, and confidence that so often follows, along with the strain on loved ones who suddenly have to rearrange their lives to help.
 
These events rarely come out of nowhere. For months, people like Beth have often been feeling a little weaker, walking less, or noticing more fatigue, and those minor warnings get brushed off as “just getting older” or labeled as “anxiety” by both patients and doctors.
From experience, I have seen that the people who keep moving, eat whole foods, and stay engaged are far more likely to enjoy longer, more fulfilling lives, yet the system rarely makes space to protect those habits. I grew tired of watching the same story repeat.
 
I often felt mainly frustrated and a bit powerless, like working on a conveyor belt. That is not how I am wired. I am a problem solver and excellent at creating unique, tailored plans. Rehab is a team effort, and I depended on and was grateful for social workers, case managers, nurses, and dietary staff. Tailored plans would upset administrative case managers because I was not following the usual script. Additionally, many patients, like Beth, asked me to stay on as their doctor because they felt seen and supported, but the system restricted that. It was not one big moment that changed me, but an accumulation of minor harms my patients had to endure while I was expected to sign off and move on. I got tired of my plans being questioned and unable to continue helping my patient when they asked.

I had to change; take a leap of faith, but I knew what to do. I drew on my medical school training in Mexico, where house calls and long-standing relationships were regular. I also drew on my public health background in prevention. I was trained to do both social visits and sick visits as house calls. Families were always so grateful when the doctor came to them. I fell in love with that style of medicine, where you know people’s stories, see their homes, and can notice small changes before they become big problems. My public health training taught me to think in terms of outcomes and prevention. Therefore, fixing the minor issues at the source prevents the entire chain of adverse events from ever happening.
I chose “longevity medicine” because it felt familiar. Behind the modern viewpoint and new tools, the core idea is very old: high-touch, compassionate, preventive care that protects your function over time. I realized I needed to bring together what I have seen work in traditional relationship-based care with prevention.
